Beginning Tuesday December 14th, per NYC’s updated vaccination mandate, MPHC will require children ages 5-11 years old to provide proof of at least one vaccination dose.

Acceptable proof of vaccination includes a CDC issued vaccination card, the NY State Excelsior Pass, and NYC Covid Safe App.
